A statement, under penalty of perjury, affirming a good faith belief that the material was removed or disabled due to error or misidentification;

Your name, address, and telephone number; an agreement to the jurisdiction of the federal district court for the judicial district in which your address is located or, for non-U.S. addresses, any district where the service provider may be found; and acceptance of service of process from the complainant.
